Your soil looks muddy, poor draining and over watered. Whats in it?
What are you watering with, plain tap water? bottled water? some crap fertilizer you found under the sink?
Do you have any means of checking the ph of your water and/or runoff?
Generally you're supposed to fill out this form before you post, or in your first post in the diag forum:

#1 way to muddy soil, way overwatered #2 Using tap water without knowing the pH, bad #3 the use of plant food may be, way to early. #4 if your pH is off(which it is), then the use of plant food was a super bad idea.
That's just from reading around. If I were you, and I'm not, I would start over.

your plant is drowning. Either transplant it out of that dirt into something drier, or figure out how to squeeze the water from your substrate. All the other problems are tertiary to the overwatering problem, get that fixed first.
