potting soil, verm, perlite, composted manure, urea, blood meal, tri-phosphorus...anything else you would add? ratios?

Blood meal: (12-0-0) Urea: (45-0-0) Tri-P: (0-46-0)
...just realized I might have more N than I need. which would you use: urea, or blood? any recommendations for a good potassium source?

Blood. I'm not sure that urea contains mobile N...and if its not, it'll only be available when you don't need it.

Potash is great for potassium! You can buy it in packets or make it yourself. Burn some hardwood (if none use any dry wood) in the yard and after the fire, collect all the white suspended ash from the top of pile.
