Overwatering.

Pick up the pot and make sure the soil is dry before you water again.

That plant is a little small to be in that big of a pot.

Also, I've strayed away from FFOF because the consistancy is just not there.

Some bags are very compact and don't allow proper drainage. Maybe mix some verm or perlite into the mix.
