Well no, not the only.
Others include:- Strain on one's financial situation
- Can't smoke in restaurants
- Smoke pollution in the air ...
- etc.
However, the vast benefits far outweigh any of the negatives.

Psh, I've smoked in front of cops, on T.V. and all through downtown. Cops never fucked with me, so, there are no negative effects to me.
